Consent to request for a report from a third party

The report is confidential and is seen only by staff in the Occupational Health Service who are dealing with your case. It does not go to any other party without your explicit and additional consent; this includes to other medical practitioners. Your consent is sought for the Occupational Health Service to apply to your General Practitioner and/or Consultant for a report giving information about your state of health. You are not obliged to give this consent but if you do not you should understand that any decision that might have to be taken on behalf of your employer will have to be taken on those facts that are readily available.

General Data Protection Regulation  (GDPR) 2018 – You or your authorised representative may apply for access to your own occupational health records under the GDPR 2018 by applying in writing to Rockingham Occupational Health Limited.  For more information on GDPR see the bottom of the page.

The Access to Medical Reports Act 1988 – This Act does not apply in this situation, as the doctor who examines you is not responsible for your clinical care.  He is acting as an independent medical adviser to your current (or prospective) employer.  For more information on Access to Medical Records see the bottom of the page.
